Applying to the Fellowship


If you are interested in applying to the Canyon Vista Medical Center Emergency Medicine fellowship, please contact our program administrator, Lorie Luse, and submit the documents listed below.

Please send all documents by mail or PDF via email. Mrs. Lorie Luse will be your point of contact for all issues related to the fellowship. Please see the application deadlines below for respective start dates.


REQUIRED DOCUMENTS

  • Personal Statement addressing your motivation for pursuing this fellowship and career goals

  • Three letters of recommendation: (one of which must be from your Residency Program Director and one should be from an emergency medicine physician who supervised you during an emergency medicine rotation during residency).

  • All letters of recommendation should include the letter writer's perception of 1) your potential and ability to successfully complete a post-graduate emergency medicine fellowship and 2) your potential and suitability for a career in Emergency Medicine

  • Medical school transcripts

  • In-training/In-service exam scores

  • USMLE/COMLEX scores

  • Residency Diploma

  • Medical School Diploma

  • ECFMG certificate (if applicable)


PRIOR TO STARTING THE FELLOWSHIP

  • You must be board certified (not board eligible) in family medicine or another similar primary care field.

  • You must have an Arizona Medical License and DEA prior to starting the fellowship

  • You must have ACLS and PALS active certifications (American Heart Association courses) and preferably ATLS (American College of Surgeons)


Application Deadline

JANUARY 1st EM FELLOW: Application must be complete by July 1st

JULY 1st EM FELLOW: Application must be complete by Jan 1st


Application Interview

If selected for interview, you MUST attend an in-person interview at Canyon Vista Medical Center in Sierra Vista, Arizona. Interviews normally occur 3-4 months prior to the start date of the fellowship. Travel and lodging costs for the interview are NOT covered by the fellowship program.
